Subcortical Ischemic Vascular Disease (SIVD) is a form of small vessel cerebrovascular disease affecting the deep white matter and subcortical structures of the brain. It is considered a subtype of vascular cognitive impairment and is characterized by chronic ischemia due to small vessel pathology. The term is often used interchangeably with subcortical ischemic vascular dementia or small vessel disease-related vascular cognitive impairment. It primarily involves damage to small penetrating arteries leading to lacunar infarcts, white matter lesions, and microvascular changes.
