Butterbur (Petasites hybridus) is an herb used in some supplements, historically promoted for migraine and allergic rhinitis. Safety is a major concern because butterbur naturally contains pyrrolizidine alkaloids (PAs), which can cause serious liver damage and are potentially carcinogenic; only products processed to remove PAs (labeled “PA-free”) have been studied, and even then rare liver injury has been reported.
